Job summary

Addus HomeCare, Inc. is seeking a compassionate Occupational Therapist (OT) to join our Home Health team in Dickson and Surrounding counties on a PRN basis. This role involves providing one-on-one care to help patients regain independence and enhance their quality of life in their own homes.

As part of our supportive and team-oriented culture, you will develop treatment plans, maintain clinical documentation, and collaborate with healthcare professionals. Ideal candidates will have a valid OT license, NBCOT registration, and at least two years of clinical experience.

Qualifications

• Graduate of an accredited Occupational Therapy program approved by AOTA.
• Current registration through NBCOT and licensed in Tennessee.
• At least two years of clinical experience as an Occupational Therapist preferred.

Responsibilities

• Provide occupational therapy services per physician’s plan of care.
• Develop individualized treatment plans to improve patient independence.
• Maintain accurate clinical documentation, including patient status updates.

What You’ll Do

• Provide occupational therapy services in accordance with the physician’s plan of care and state practice guidelines
• Develop individualized treatment plans and establish functional goals to improve patient independence
• Communicate plans of care with physicians and interdisciplinary team members
• Teach new skills or retrain patients in activities of daily living impacted by illness or injury
• Fabricate splints and instruct patients in the use of adaptive equipment to enhance function
• Train patients in the use of prosthetic and orthotic devices as appropriate
• Maintain accurate clinical documentation, including notes, reports, and updates to patient status
• Communicate changes in patient condition to the physician and care team
• Collaborate with other healthcare professionals to ensure coordinated and efficient patient care

Qualifications

• Graduate of an accredited Occupational Therapy program approved by the American Occupational Therapy Association (AOTA)
• Current registration through the National Board for Certification in Occupational Therapy (NBCOT) and licensed in the state of practice
• At least two (2) years of clinical experience as an Occupational Therapist (preferred)
• Current CPR certification
• Reliable transportation, valid driver’s license, and current automobile insurance
