Job summary

Creative Solutions In HealthCare seeks a compassionate Occupational Therapist (OT) for a PRN role within our in-house rehab team. You will evaluate residents, develop personalized treatment plans, and support independence and safety through ADLs and IADLs.

The ideal candidate holds an active Texas OT license, has experience in SNF or long-term care, and collaborates with an interdisciplinary team. Competitive compensation and flexible schedules are offered.

Qualifications

• Active and valid Texas Occupational Therapy license.
• In good standing with licensing boards.
• SNF or long-term care experience preferred.
• Knowledge of occupational therapy treatment practices.
• Experience with Net Health EDS preferred.
• Ability to manage diverse patient needs.
• Strong communication and collaboration skills.

Responsibilities

• Consult with patients to evaluate physical condition and impact on daily activities.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADLs and IADLs.
• Assess home environments and recommend modifications based on needs.
• Recommend adaptive equipment and provide training for independence and safety.
• Ensure treatments align with the established plan of care.
• Monitor patient response and adjust interventions with the interdisciplinary team.
• Provide education to patients, families, and caregivers.
• Collaborate with interdisciplinary team to ensure quality care.
• Complete documentation per policies and regulations.
• Attend required meetings as designated by the Director of Rehab.
