Job summary

Continuum Therapy Partners, LLC is seeking a licensed Occupational Therapist for PRN shifts at Evergreen Nursing & Rehab in Effingham, IL. You’ll provide high-quality, patient-centered therapy with the same standards as our full-time team.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
