Responsibilities

• Screen and evaluate patients with cognitive and/or ADL disorders.
• Utilize assessments to support clinical interventions.
• Provide medically necessary treatment interventions.
• Administer parts of the rehabilitation plan.
• Assess patient progress and modify treatments accordingly.
• Evaluate treatment outcomes for effectiveness.
• Consult and counsel patients, families, and caregivers.
• Communicate with supervisor and health team members.
• Provide comprehensive discharge summaries.
