Occupational Therapist – PRN

Palomar Vista Healthcare Center – Escondido, CA

Palomar Vista Healthcare Center is a post-acute care facility with long-term care, short-term care, and geriatric outpatient services. We are seeking a PRN Occupational Therapist to join our in-house therapy team. As part of this team, you will have opportunities for training and continuing education in Think Thin/dysphagia, Abilities Care and dementia, stroke and neuro rehabilitation, and respiratory recovery programs.

Key Responsibilities

• Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop an appropriate plan of care following all regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan including ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc.
• Assess patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness. Modify services in relation to patient evaluation and make referrals based on outcomes.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to the physical disorders.
• Effectively communicate with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provide a comprehensive discharge summary of services provided per regulatory and clinical requirements.
