Job summary

Greenridge Post-Acute and Senior Living in El Sobrante, CA is seeking a licensed Occupational Therapist on a PRN basis. You will have control over your schedule, premium hourly compensation, and a collaborative rehabilitation team that values your clinical expertise.

You will evaluate and develop individualized plans of care, lead OT sessions, supervise assistants, and coordinate discharge planning to support resident independence and quality of life.

Qualifications

• CA OT license required (or pending licensure)
• NBCOT certification required
• Graduation from an accredited OT program
• Current CPR/BLS certification
• Experience in skilled nursing or post-acute care preferred
• Excellent communication and clinical judgment

Responsibilities

• Conduct comprehensive OT evaluations and re-evaluations.
• Develop, implement, and modify individualized plans of care with measurable goals.
• Deliver skilled OT interventions addressing ADLs, IADLs, UE rehab, cognition.
• Assess and recommend adaptive equipment, splints, and environmental modifications.
• Supervise OTAs and rehab staff per California state law.
• Lead discharge planning and coordination for home/community reintegration.
• Document findings and progress accurately and timely.
• Collaborate with physicians and interdisciplinary team across care settings.
