Responsibilities

• Evaluate residents per MD order and establish Treatment Plans and Goals.
• Re-assess patients and caseload to determine need for continued services or discharge.
• Provide thorough discharge planning to maximize independence in the least restrictive environment.
• Identify equipment needs and order equipment per policy.
• Complete documentation per Guidelines in Documentation Manual.
• Coordinate staff in-services to ensure safe and effective carry over of programs.
• Attend Continuing Education courses relevant to the position.
• Maintain professionalism and follow all relevant regulations.
