Job summary

Continuum Therapy Partners, LLC seeks an Occupational Therapist to support Admiral’s Pointe in Huron, Ohio. This PRN daytime role emphasizes patient-centered OT care within a therapist-led team.

The OT will evaluate, plan, and document progress toward functional independence, and educate patients and families on adaptive equipment and home safety. Licensure and NBCOT certification are required; Master’s/ Doctorate in OT preferred.

Qualifications

• Master’s or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
