Job summary

Continuum Therapy Partners, LLC is seeking an Occupational Therapist to evaluate, plan, and implement patient-centered therapy programs. The role emphasizes advancing functional independence and educating patients and families on adaptive equipment and home safety, with periodic progress documentation.

Preferred candidates hold an OT Master’s or Doctorate, an active state license, and NBCOT certification, with at least 1 year of related experience.

Qualifications

• Master’s or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
