Job summary

Live Well Homecare is seeking an Occupational Therapist on a PRN basis to provide skilled OT services in home settings. You will evaluate patients, plan and implement treatments, and collaborate with families and the care team to determine needs and referrals.

You will educate stakeholders, document care accurately, and adapt to weekend or alternate hours as needed to support patient evaluations and ongoing care.

Qualifications

• Bachelor's Degree in Occupational Therapy (OT) is required.
• OT licensure is required.
• Valid Driver's License is required.
• Strong collaboration and clear communication with patients and teams.
• Master's in OT is preferred.

Responsibilities

• Develop and implement treatment plans considering discharge needs and equipment.
• Educate and consult with patients, families, and healthcare providers.
• Delegate tasks to assistants and students based on skill levels.
• Complete accurate documentation of services provided.
• Conduct assessments, reassessments, and consultations for occupational therapy.
