Job summary

Crossover Health is seeking a PRN Nurse Practitioner for our Durham Fidelity Primary care clinic. Availability during M-F 8am-5pm is required to cover 8-hour shifts.

The NP delivers patient-centered primary care, collaborates with a multidisciplinary team, and supports quality improvement initiatives to maintain high standards of care. Responsibilities include supervising staff, selecting treatment plans, prescribing treatment options, and referring patients as needed.

Qualifications

• 5+ years of clinical experience in Internal Medicine or Family Practice
• Current Board Certification
• Licensed to practice in the appropriate state and current BLS certification

Responsibilities

• Provides patient centered, comprehensive primary care services.
• Participate in selecting suitable treatment plans for all patients.
• Supervise staff to ensure the care team provides appropriate care.
• Assist other care providers and extended care teams.
• Provide continuous availability for medical and technical questions to employees and staff.
• Participate in quality improvement and multidisciplinary quality assurance programs.
• Collaborate with management to improve patient care.
• Cooperate with clinical outcome reviews and measurements.
• Comply with policies, procedures, and protocols.
• Provide general medical care under physician direction.
• Submit health care plans and goals for periodic review.
• Prescribe or recommend drugs or other treatments.
• Refer patients to physicians or specialists as needed.
• Perform other duties as assigned.
