Job summary

Crossover Health is seeking a Nurse Practitioner to deliver high-quality patient-centered primary care within our Primary Health model. The position emphasizes an interdisciplinary care team approach and care delivered in-person, virtually, and via messaging.

The role requires 5+ years in Internal Medicine or Family Practice, board certification, and state licensure, plus BLS. Base pay ranges from $75.12 to $98 per hour with comprehensive benefits.

Qualifications

• Minimum of 5 years of clinical experience in Internal Medicine or Family Practice.
• Must be Board Certified and licensed to practice in the appropriate state.
• BLS (Basic Life Support) certification required.
• May refer patients to physician or to specialized health resources as needed.

Responsibilities

• Provides patient centered, comprehensive primary care services.
• Supervise staff to ensure the care team provides appropriate care.
• Assist other care providers and extended care teams.
• Provide continuous availability for medical and technical questions to employees, patient care staff, and ancillary care providers.
• Participate in quality improvement programs and multidisciplinary quality assurance.
