Job summary

Crossover Health in Durham Fidelity Primary care clinic is seeking a PRN Nurse Practitioner to provide patient-centered primary care within our clinic schedule, typically covering 8-hour shifts during M-F 8am-5pm.

The Nurse Practitioner will meet clinical, administrative, and financial performance metrics and deliver high-quality care under physician oversight, collaborating with an interdisciplinary care team.

Qualifications

• Minimum of 5 years of clinical experience in Internal Medicine or Family Practice.
• Board Certified and licensed in the appropriate state.
• Current BLS certification.

Responsibilities

• Provides patient-centered, comprehensive primary care services.
• Participate in selection of suitable treatment plans for all patients.
• Supervise staff to ensure the care team provides appropriate care.
• Assist other care providers and extended care teams.
• Provide ongoing availability for medical and technical questions to employees and patients, including after-hours coverage.
• Participate in quality improvement programs and multidisciplinary quality assurance programs.
• Collaborate with management to improve patient care.
• Cooperate with clinical outcome reviews and measurements.
• Comply with policies, procedures, and protocols.
• Provide general medical care and treatment under physician direction.
• Submit health care plans and goals for periodic review.
