Job summary

PruittHealth Hospice is seeking a PRN Nurse Practitioner for Hospice & Palliative Care to provide comprehensive assessments and manage plans of care in patients' homes or care facilities.

The NP will work collaboratively with an interdisciplinary team, complete documentation accurately, and adhere to established policies to ensure continuity of care for hospice patients.

Qualifications

• Current, active, and unrestricted RN and NP licensure in state of practice.
• Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care.

Responsibilities

• Proactive, collaborative team member in a hospice care environment.
• Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care.
• Commitment to caring for patients and partners.
• Complete all documentation and records in a timely and accurate manner.
• Follow established policies and procedures as related to the scope of practice.
• Maintain continuity of patient care in a hospice setting.
