Job summary

Qualifications

• Active RN license in state of practice and NP license must be current and unrestricted.
• Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care.

Responsibilities

• Proactive, collaborative team member in a hospice care environment.
• Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care.
• Commitment to caring for patients and partners.
• Complete all documentation and records in a timely and accurate manner; follow established policies and procedures related to scope of practice.
• Maintain continuity of patient care in a hospice setting.
