Job summary

PruittHealth Hospice is seeking a Nurse Practitioner to provide hospice and palliative care in patients' homes or care facilities. This role focuses on comprehensive assessment, symptom management, and care coordination within a multidisciplinary team.

You will conduct adult/geriatric evaluations, develop and adjust plans of care, and collaborate with nurses, physicians, and social workers to deliver compassionate, high-quality care.

Qualifications

• Current, active, and unrestricted RN and NP licensure in state of practice.
• Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care.

Responsibilities

• Proactive, collaborative team member in a hospice care environment.
• Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care.
• Commitment to caring for patients and partners.
• Complete all documentation and records in a timely and accurate manner.
• Follow established policies and procedures as related to the scope of practice.
• Maintain continuity of patient care in a hospice setting.
