Job summary

HarmonyCares is seeking a Nurse Practitioner to perform annual risk assessments in a home setting or via telehealth, under supervision of a Collaborative Physician. The role centers on comprehensive in-home evaluations and accurate documentation.

Ideal candidates hold an active NP license, board certification (ANCC/AANP/NCCPA), BLS, and a valid driver’s license with the ability to travel for home visits. Excellent EHR skills and strong communication are essential.

Qualifications

• Active NP license required and unrestricted in practice state(s).
• Board certification by ANCC, AANP, or NCCPA preferred.
• BLS certification required.
• Proficient in Electronic Health Records (EHR) systems.
• Valid driver’s license with clean driving record.
• Willingness to travel for in-home visits (up to ~150 miles daily).

Responsibilities

• Conduct comprehensive in-home health risk assessments and identify active/chronic conditions.
• Take patient history, perform examinations, determine diagnoses.
• Provide written visit documentation per NCQA standards.
• Record vital signs and initiate case management referrals as needed.
• Coordinate with PCPs, caregivers, and other care team members.
• Maintain patient confidentiality and OSHA safety practices.
• Participate in meetings, trainings, and professional development.
• Collaborate with patients and families to support care plans.
