Job summary

Hueman is seeking Nurse Practitioners to perform in-home Health Risk Assessments for Medicare and Medicaid members around New Philadelphia, OH. This role offers flexible scheduling and a meaningful way to contribute to community health outside traditional clinics.

Qualified NPs with an active state license and board certification will travel to patients' homes, perform non-invasive assessments, and document encounters. Training and liability coverage are provided.

Qualifications

• Master's degree in nursing required.
• Active State NP license.
• Active National board certification.
• Current BLS certification.
• 12 months of experience as a paid advanced clinical provider is preferred but not required.

Responsibilities

• Travel to patients' homes to perform HRAs.
• Conduct comprehensive, non-invasive physical assessments.
• Review medical history and document findings accurately.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative care measures.
