Job summary

Paris Regional Medical Center is seeking a Social Worker, Bachelors, to provide social services and discharge planning for inpatients, outpatients, and ED patients. This PRN role supports the hospital’s care transitions and CQI activities, ensuring resources are available when needed.

The ideal candidate holds a Bachelor’s in Social Work (or related field), has 2+ years of professional social work experience, and at least 1 year in a medical setting with discharge planning experience.

Qualifications

• Bachelor's Degree required in Social Work, Psychology, Sociology or related field.
• 2+ years of professional social work experience.
• 1 year experience in a medical setting.
• Previous experience in all aspects of social service and discharge planning.

Responsibilities

• Provides social service and discharge planning functions for the hospital.
• Provides resources for inpatients, outpatients, and Emergency Department patients.
• Supports the skilled unit designee when resources are unavailable.
• Maintains performance improvement activities within the department and participates in CQI activities.
