Job summary

Baptist Health Downtown campus in Jacksonville, FL is seeking a Master's level Social Worker for a PRN Days role. The successful candidate will join an interdisciplinary care team to support patients’ psychosocial needs, coordinate discharge planning, and connect patients with appropriate community resources.

Responsibilities include assessing psychosocial needs, arranging home health and DME, counseling, facilitating placements and transfers, and providing referrals to community services.

Qualifications

• Master's degree in Social Work required.
• 1-2 years social work experience.
• 3-5 years discharge planning experience.
• Home health experience preferred.

Responsibilities

• Assessing psychosocial needs and provides services that enhance patient functioning, emotional well-being and continued access to services and resources in the community.
• Provides discharge planning, arranges home health and DME
