Job summary

Nurturing Home Healthcare is seeking a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in their homes on a PRN, per-visit basis in the Houston, TX area.

Under the supervision of an RN and in collaboration with physicians and care teams, you will administer medications, monitor conditions, perform wound care, educate patients and caregivers, and maintain accurate documentation while adhering to Medicare guidelines and agency policies.

Qualifications

• Active LVN license in Texas.
• Minimum 1 year of nursing experience.
• Knowledge of nursing procedures and med administration.
• Excellent verbal and written communication.
• Ability to work independently and manage visits efficiently.
• CPR certification or willingness to obtain on hire.

Responsibilities

• Provide skilled nursing per physician orders and care plans.
• Monitor patient conditions and report changes to RN/physician.
• Administer medications and treatments safely.
• Perform wound, ostomy, catheter care as permitted.
• Educate patients and caregivers on meds and disease management.
• Document visits, observations, and outcomes accurately.
• Collaborate with RNs, physicians and therapists.
• Adhere to infection control and confidentiality.
