Job summary

Nurturing Home Healthcare is seeking a compassionate Licensed Vocational Nurse (LVN) for a PRN home health role in Houston, TX. You will provide skilled nursing care under RN supervision, following physician orders and individualized care plans to support patient independence at home.

Ideal candidates will have 1+ year of nursing experience, strong clinical assessment skills, and reliable transportation. Wound care, med administration, documentation, and teamwork with healthcare professionals

Qualifications

• Current Texas LVN license active.
• Minimum of 1 year of nursing experience.
• Knowledge of basic nursing procedures and medication administration.
• Strong verbal and written communication skills.
• Ability to work independently and manage patient visits efficiently.

Responsibilities

• Provide skilled nursing services to home health patients per physician orders and care plans.
• Monitor and assess patient conditions, reporting changes to RN/physician.
• Administer prescribed medications and treatments safely and accurately.
• Perform wound, catheter, ostomy care, and other nursing procedures as licensed.
• Educate patients and caregivers on medications, disease management, safety, and treatment plans.
• Document visits, observations, interventions, and outcomes accurately and timely.
• Coordinate patient care with RNs, physicians, therapists, and other professionals.
• Ensure infection control practices and patient confidentiality.
