Job summary

Nurturing Home Healthcare is seeking a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care in patients' homes as a PRN role in Houston, TX. You will work under RN supervision and coordinate with physicians to ensure quality care and independence for patients.

Responsibilities include medication administration, wound and ostomy care, patient education, and timely documentation. Travel to patient homes is expected; CPR may be required.

Qualifications

• Current and active LVN license in Texas.
• Minimum of one year of nursing experience.
• Strong clinical assessment and patient care skills.
• Knowledge of basic nursing procedures and medication administration.
• Excellent verbal and written communication skills.
• Ability to work independently and manage visits efficiently.
• Reliable transportation with valid driver's license and proof of auto insurance.
• Current CPR certification or ability to obtain upon hire.
• Ability to maintain accurate and timely clinical documentation.
• Strong organizational and time-management skills.

Responsibilities

• Provide skilled nursing services to home health patients per physician orders and care plans.
• Monitor and assess patient conditions, report changes to supervising RN/physician.
• Administer prescribed medications and treatments safely and accurately.
• Perform wound, catheter, ostomy care and other procedures as allowed by licensure.
• Educate patients and caregivers on medications, disease management, safety and treatment plans.
• Document visits, observations, interventions and outcomes accurately and timely.
• Assist with implementation and monitoring of care plans.
• Collaborate with RNs, physicians, therapists and other health professionals.
