Job summary

Paradigm Home Health Companies seeks a PRN Licensed Vocational Nurse to provide direct patient care to home-bound patients. You will perform wound care, administer medications, and support plans of care in coordination with the interdisciplinary team.

Qualities include Texas LVN licensure, BLS/CPR certification, and a valid Texas driver license. Proficiency with WellSky EMR is a plus. Pay is on a per-visit basis.

Qualifications

• Advocates for patient care and suggests ways to improve quality of life.

Job description

PRN Licensed Vocational Nurse will provide direct patient care to home bound patients.

Responsibilities and Duties:

• Provides wound care, disease process education, medication administration, and other clinical duties as prescribed by NP's, PA's, and Physicians.
• Carries out the clinical plans of care to assist patients maintain their independence.
• Coordinates with the interdisciplinary clinical team regarding patient changes in condition and proactively communicates patient's clinical needs.
• Provides education to patients and families on proper home health care procedures and strategies.
• Advocates for appropriate patient care and recommends strategies to help improve the patient's quality of life.

Qualifications and Skills:

• Current Texas LVN License.
• Current Certification in BLS/CPR
• Valid Texas Driver's License and Automobile Insurance
• Knowledge of WellSky EMR software is a plus
