Job summary

Qualifications

Responsibilities

• Provides wound care, disease process education, medication administration, and other clinical duties as prescribed by NP/PA/Physicians.
• Carries out the clinical plans of care to assist patients maintain their independence.
• Coordinates with the interdisciplinary clinical team regarding patient changes in condition and proactively communicates patient's clinical needs.
• Provides education to patients and families on proper home health care procedures and strategies.
• Advocates for appropriate patient care and recommends strategies to improve the patient's quality of life.

• Carries out the clinical plans of care to assist patients maintain their independence.
• Coordinates with the interdisciplinary clinical team regarding patient changes in condition and proactively communicates patient's clinical needs.
• Provides education to patients and families on proper home health care procedures and strategies.
• Advocates for appropriate patient care and recommends strategies to help improve the patient's quality of life.
