Job summary

Paradigm Home Health Companies in Texas seeks a Licensed Vocational Nurse to provide direct patient care for home bound clients, including wound care, education, and medication administration according to physician orders.

You will implement plans of care, coordinate with the interdisciplinary team, and communicate changes in condition while advocating for patient quality of life and independent living where possible.

Qualifications

• Current Texas LVN License.
• Current Certification in BLS/CPR
• Valid Texas Driver's License and Automobile Insurance
• Knowledge of WellSky EMR software is a plus
