Job summary

Paradigm Home Health Companies seeks a PRN Licensed Vocational Nurse to provide direct patient care to home-bound patients in the North Ft. Worth area. This role requires compassionate clinical support and flexible scheduling.

Responsibilities include wound care, medication administration, teaching families, and coordinating with the care team to adapt plans as patients' needs change. A valid Texas LVN license and CPR/BLS certification are required.

Qualifications

• Current Texas LVN License.
• Current Certification in BLS/CPR.
• Valid Texas Driver's License and Automobile Insurance.
• Knowledge of WellSky EMR software is a plus.

• Carries out the clinical plans of care to assist patients maintain their independence.
• Coordinates with the interdisciplinary clinical team regarding patient changes in condition and proactively communicates patient's clinical needs.
• Provides education to patients and families on proper home health care procedures and strategies.
• Advocates for appropriate patient care and recommends strategies to help improve the patient's quality of life.
