Big Horn Rehabilitation and Care Center, now under Eduro Healthcare LLC, seeks an experienced Licensed Practical Nurse (LPN) to join the long-term care unit in Sheridan, Wyoming. Qualifications

• Active Wyoming LPN/LVN license required.
• BLS certification from the AHA required.
• IV certification preferred.

Responsibilities

• Provide comprehensive Licensed Practical Nurse (LPN) care for residents.
• Administer medications and treatments as prescribed by the attending physician and in accordance with facility protocols.
• Monitor residents’ health status and promptly report changes to the supervising RN, physician, and families.
• Provide skilled nursing services within scope of practice, including tube feedings, suctioning, catheter care, wound care, dressing changes, and other ordered treatments.
• Accurately complete all required documentation, charting, and reports per regulatory requirements and facility policies.
• Support individualized resident care plans while respecting resident dignity, preferences, and rights.
• Observe residents closely and report significant changes to the interdisciplinary care team.
• Collaborate with the interdisciplinary team to ensure high-quality resident care.
