Job summary

Qualifications

• Education must be from an approved Practical/Vocational Nursing program.
• One year of qualifying experience may be required for higher grades.
• GS-03 requires basic licensure and scope under supervision.

Responsibilities

• Provide direct patient care in Primary Care within PACT.
• Assist with care coordination and patient education.
• Participate in PACT huddles and performance measures.
• Coordinate care for patients including those seen via Clinical Video Tele-Health.
