Responsibilities

• Provide direct Veteran care and care coordination in Primary Care.
• Manage patient panels and utilize registries for appropriate evaluation and access.
• Deliver health promotion, wellness coaching, and disease prevention.
• Collaborate to develop, implement, evaluate, and revise policies and guidelines.
• Support peers through knowledge sharing and ensure safe, quality care.
