Position Overview:
Nest & Care is currently seeking an experienced Licensed Practical Nurse (LPN) for a PRN position performing home health visits, client assessments, and caregiver training for our clients in and around Montgomery County, MD.
This position is ideal for a Licensed Practical Nurse (LPN) who is passionate about providing exceptional home care to clients. Responsibilities:
- Assisting in assessing, planning, implementing, and evaluating patient care;
- Conducting and documenting ongoing data collection related to patient/family reporting to the RN following guidelines, policies, and procedures;
- Providing care to the age-specific patient populations such as children, adolescents, adults or older adults utilizing the nursing process;
- Providing direct or indirect patient care services including but not limited to medication administration;
- Demonstrating the ability to perform and respond appropriately in emergency situations;
- Completing nursing care documentation for assigned patients;
- Developing and maintaining effective communication with patients, families, physicians, colleagues, and co-workers, to ensure the ongoing provision of patient care;
- Performing tasks and/or interacting with patients, families, staff, and other personnel in a courteous, honest and compassionate manner;
- Observing patients, charting and reporting changes in patients' conditions, such as adverse reactions to medication or treatment, and taking any necessary action;
- Administering prescribed medications or starting intravenous fluids, and noting times and amounts on patients' charts;
- Answering patients' calls and determining how to assist them;
- Measuring and recording patients' vital signs, such as height, weight, temperature, blood pressure, pulse, and respiration;
- Providing basic patient care and treatments, such as taking temperatures and blood pressure, dressing wounds, treating bedsores, giving enemas, douches, alcohol rubs, and massages, or performing catheterizations;
- Helping patients with bathing, dressing, and personal hygiene, moving in bed, and standing and walking;
- Working as part of a healthcare team to assess patient needs, plan and modify care and implement interventions;
- Recording food and fluid intake and output;
- Evaluating nursing intervention outcomes, conferring with other healthcare team members as necessary.
Qualifications:
- Graduate of an accredited school of nursing, or proof of successful challenge of LPN Exam;
- Minimum of one (1) year of recent experience in a home care setting for pediatric, adolescent, and geriatric patients, within the period of two (2) years;
- Current unrestricted LPN license in the State of Maryland;
- Current IV Certificate;
- Current CPR and First Aid certification;
- TB vaccination and Hep B vaccination;
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma;
- Clear criminal background and driving record;
- Two professional references within the healthcare field;
- Prior home care experience is strongly preferred;
- Self-starter with the ability to work independently and manage time effectively;
- Strong interpersonal skills, including a willingness to teach and mentor caregivers;
- Comfort with technology; prior experience with electronic medical records preferred;
- Ability to drive and travel to cases as assigned;
- Availability for weekdays and some weekends.
