Job summary

Avamere Rehab of Lebanon in Lebanon, OR is seeking a Licensed Practical Nurse to provide essential nursing care under the supervision of an RN. The role includes vital signs, medication administration, wound care, and assisting residents with daily activities.

The LPN will monitor patient status, document observations, and communicate with residents, families, and other healthcare professionals. Collaboration with CNAs and a strong emphasis on safety are essential.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, unencumbered, active LPN license to practice in this state.
• Must maintain and have an active CPR/BLS during employment.
• Prefer experience in long term care, geriatric rehab, sub-acute or critical care settings.
• Experience in electronic medical records documentation.

Responsibilities

• Provide basic nursing care under RN supervision.
• Accurately measure and record vital signs.
• Administer medications as prescribed, following facility protocols.
• Perform wound care and monitor for signs of infection.
• Assist residents with hygiene, dressing, bathing, and mobility.
• Ensure proper nutrition and hydration, assist with feeding if needed.
• Record patient history and observe changes, reporting to RN.
• Communicate with residents, families and healthcare professionals.
• Collaborate with RN and CNAs to deliver comprehensive care.
• Maintain patient safety and document all care provided.
