Job summary

Avamere Transitional Care at Sunnyside in Salem, OR is hiring a Licensed Practical Nurse to provide basic nursing care under the supervision of an RN. The role includes vital signs, medication administration, wound care, hygiene, and mobility assistance, with a focus on resident comfort and safety.

Requirements include an active LPN license in Oregon, CPR/BLS, and a nursing degree from an accredited program. Experience in long-term care settings is preferred and EMR experience is a plus.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, unencumbered, active license to practice as an LPN in this state.
• Must maintain and have an active CPR/BLS during employment.
• Experience in long term care, geriatric rehab, sub-acute or critical care setting preferred.
• Experience in electronic medical records documentation.

Responsibilities

• Provide basic nursing care including vital signs, medication administration, wound care, and assisting residents with daily activities under RN supervision.
• Support residents and families with care and comfort.
• Record patient history and current condition; observe and report changes to RN.
• Communicate effectively with residents, families and healthcare professionals.
• Supervise CNAs when applicable and collaborate with the care team.
• Maintain patient safety and document all care provided.
