Job summary

Addus HomeCare in Columbia, SC seeks a PRN LPN to provide nursing support in clients' homes under RN supervision. You will perform assessments, medication set ups, and health evaluations while promoting independence and adherence to care plans.

Qualifications

• Graduation from an accredited school of nursing and current LPN licensure in the state of practice.

Responsibilities

• Medication set ups and reviews
• Skin assessments and general health evaluations
• Basic nail care
• Client education and documentation
• Enable patients to stay independent while they remain in their own homes
• Perform routine supervisory visits
• Follow up for unskilled services
• Maintain confidentiality and comply with Medicaid/HIPAA regulations
• Maintain regular attendance and flexibility to meet agency needs
• Abide by all policies and standards

We are seeking a PRN LPN to provide nursing support to clients within our agency This role is critical to meeting current care plan requirements skilled nursing in a home seeting The Licensed practical nurse performs assessments and simple medical nursing procedures in clients homes under the guidance of the RN Supervisor in accordance with physicians orders Quality patient care is a top priority and employees are recognized as our greatest asset We respect your desire for independence and flexibility in your work and personal life and will design a FLEXIBLE SCHEDULE to meet your needs Hours self scheduled during the week Monday through Friday

Responsibilities and Duties

• Medication set ups and reviews
• Skin assessments and general health evaluations
• Basic nail care
• Client education and documentation
• Enable patients to stay independent while they remain in their own homes
• Perform routine supervisory visits
• Follow up for unskilled services
• Maintain a high degree of confidentiality at all times due to access to sensitive information
• Maintain regular predictable consistent attendance and is flexible to meet the needs of the agency
• Follow all Medicaid and HIPAA regulations and requirements
• Abide by all regulations policies procedures and standards
• Performs other duties as assigned

Position Requirements & Competencies

• Graduation from an accredited school of nursing and current LPN licensure in the state of practice
• New Nurses welcome will train
• Ability to make home visits and provide nursing care in the home environment
• No weight restrictions
• Home Care experience preferred but not mandatory
• Must have reliable transportation current drivers license and appropriate automobile insurance; or ability to take public transportation
• Ability to complete necessary documentation appropriately legibly and timely
• Interpersonal verbal and written communication skills necessary to complete job responsibilities
