Job summary

Summa Home Health seeks a Licensed Practical Nurse to provide skilled nursing care in patients’ homes across the Akron area. You’ll perform nursing tasks, monitor conditions and administer medications under physician orders, while coordinating with families and the care team.

Applicants should have LPN licensure in the state, 1+ year of LPN experience (2+ preferred), and home health experience is a plus. Strong communication and compassion are essential.

Qualifications

• Must hold an LPN license in the state of employment.
• Minimum of one year of LPN experience in an acute care setting is required; two years preferred.
• Home health experience is desired.
• Ability to effectively communicate with patients, families, physicians and coworkers.
• Calm, patient, accommodating demeanor.
• Detail-oriented with sound nursing judgment.
• Valid driver's license and auto insurance in your name.

Responsibilities

• Provide skilled nursing care in patients' homes as ordered by the attending physician.
• Follow order sets and goals established by RN.
• Observe and monitor patient conditions; notify RN/CM of changes.
• Administer medications as prescribed; counsel patient and family on disease processes and management.
• Develop and implement plan of care with patient and family input.
• Help decrease hospitalizations through preventative guidance and good decisions.
• Complete clinical documentation per agency protocol and Medicare guidelines.
• Maintain active LPN license in state of employment.

MAJOR AREAS OF RESPONSIBILITY:

• Provides skilled nursing care in a patient's home as ordered by the attending physician.
• Performs care by following order sets and goals made by RN.
• Observes and monitors patient conditions.
• Notifies RNCM of any changes in patient condition.
• Administers medication as prescribed by the physician.
• Counsels patient and family on the disease/injury processes and how to manage.
• Incorporates patient and family in development of plan of care.
• Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.
• Listens to patient and family members.
• Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
• Documents all aspects of treatment, assessments, and patient education.
• Maintains active LPN license in state of employment.
• Communicates with scheduler any changes outside the normal.
• Participates in all on-call requirements and case conferences.
• Attends in-service trainings and mandatory agency meetings.
• Maintains a professional appearance as a representative of the company.
• Understands and practices agency policies and procedures.
• Other duties as assigned.

ESSENTIAL FUNCTIONS:

• Drives to patient's primary location per scheduled visit.
• Daily attendance at assigned visit locations.
• Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines.
• Completes and submits all required documentation within specified company requirements.
• Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse.
• Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.
