Job summary

Human Touch Home Health in Blacksburg, VA is seeking a compassionate Licensed Master Social Worker (LMSW) for a PRN home health opportunity serving Blacksburg, Christiansburg, Radford and surrounding New River Valley communities.

As part of an interdisciplinary team, you will conduct psychosocial assessments, identify needs, connect families with resources, and support planning and counseling within your scope.

Qualifications

• Master's Degree in Social Work from an accredited program.
• Virginia social work licensure required.
• Valid Driver's License and reliable transportation.
• Home Health or Healthcare Social Work Experience Preferred.
• WellSky experience is a plus.

Responsibilities

• Complete psychosocial assessments in patients' homes.
• Identify social, emotional, financial, and environmental needs.
• Connect patients and families with community resources.
• Assist with long-term planning and available support services.
• Provide counseling and emotional support within scope of practice.
• Assist patients and families with navigating healthcare and community programs.
• Educate patients and caregivers on available resources.
• Collaborate with nurses, therapists, physicians, and other care team members.
• Document visits and interventions accurately and on time using WellSky.
• Support the overall plan of care and patient goals.
