Job summary

Human Touch Home Health is seeking a compassionate Licensed Master Social Worker (LMSW) for a PRN home health opportunity serving Blacksburg, Christiansburg, Radford, Pulaski, Floyd, and surrounding New River Valley communities.

As a Social Worker, you'll complete psychosocial assessments, connect patients with community resources, and provide counseling to support safe, independent living at home. Virginia licensure and a valid driver's license are required.

Qualifications

• Master's Degree in Social Work from an accredited program.
• Virginia Social Work licensure required.
• Valid Driver's License and reliable transportation.
• Strong communication, assessment, and documentation skills.
• Ability to work independently in patients' homes.
• Home Health or healthcare social work experience preferred.
• WellSky experience is a plus.

Responsibilities

• Complete psychosocial assessments in patients' homes.
• Identify social, emotional, financial, and environmental needs.
• Connect patients and families with community resources.
• Assist with long-term planning and available support services.
• Provide counseling and emotional support within scope of practice.
• Assist patients and families with navigating healthcare and community programs.
• Educate patients and caregivers on available resources.
• Collaborate with nurses, therapists, physicians, and other care team members.
• Document visits and interventions accurately and on time using WellSky.
• Support the overall plan of care and patient goals.
