Job summary

Stonecreek Health and Rehabilitation in Paducah, KY is seeking a PRN LPN to provide bedside care and support for residents in a 90-bed LTC setting. You will administer medications, monitor responses, take vital signs, and assist with comfort and mobility, while delivering compassionate, patient-centered care.

The role offers shift differentials, tuition reimbursement, and a supportive team culture where staff contributions are celebrated.

Qualifications

• Current LPN license in Kentucky.
• At least one year of LPN experience in a Long Term Care setting preferred.
• Willingness to work evenings and weekends.
• Exceptional staff training and customer service skills.
• Compassionate and friendly demeanor.
• CPR and first aid certifications required.

Responsibilities

• Provide bedside care and document therapeutic effects.
• Administer medications and monitor effects.
• Take vital signs including temperature, blood pressure, pulse and respiration.
• Assist with resident comfort and mobility.
• Prepare and administer injections and enemas.
• Monitor residents and respond to changes in condition.
• Support and coach other staff as needed.

LPN responsibilities:

• Provide bedside care. Administer medications and document therapeutic effects

• Take vital signs (temperature, blood pressure, pulse and respiration)

• Assist with resident comfort

• Prepare and administer injections and/or enemas

• Collect samples and monitor catheters

• Provide wound care

• Help coach staff as necessary

LPN qualifications:

• Up to date first aid and CPR certification

• Current LPN license in the state

• At least one year of LPN experience in a Long Term Care Setting preferred

• Willingness to work evenings and weekends

• Exceptional staff training and customer service skills

• Compassionate and friendly demeanor
