ProHealth Home Health is seeking a Licensed Physical Therapy Assistant (LPTA) for Bartow County, GA on a PRN, part-time basis. The role involves delivering physician-prescribed PT in patients' homes under the supervision of a Registered Physical Therapist.

Requirements include graduation from a two-year PTA program approved by the APTA and active licensure in the applicable state, with two years’ experience preferred and a demonstrated ability to work autonomously.

Qualifications

• Graduate from a two-year college level PTA program approved by the American Physical Therapy Association.
• Licensed, registered or certified in the state(s) where practicing.
• Two years’ experience preferred.
• Acceptance of the agency's philosophy and goals.
• Ability to exercise initiative and independent judgment.

Responsabilités

• Understand and adhere to agency policies and procedures.
• Provide physician prescribed physical therapy under the PT plan of care.
• Improve or minimize residual physical disabilities of the patient.
• Return the patient to optimum and productive level within their capabilities.
• Participate with other health care personnel in care planning.
• Perform skilled procedures as ordered by physician and per PT plan of care.
• Consult with PT regarding treatment changes.
• Instruct patients and families in home programs and activities of daily living.
• Participate in inservice programs and present as assigned.
• Participate in QAPI activities as assigned.
• Attend patient care conferences as scheduled.
• Prepare medical records, progress notes and updates to care plans timely.
• Perform services planned, delegated and supervised by the PT.
• Assist in preparing clinical and progress notes.
• Educate patient and family related to care.
