Job summary

ProHealth Home Health is seeking a Licensed Physical Therapy Assistant to administer therapy to patients in their homes in Bartow County, GA. The role is part-time and requires licensure with relevant state credentials and optional two years of experience.

The LPTA will work under the supervision of a registered PT, following a prescribed plan of care and contributing to patient recovery and independence through home-based exercises and activities of daily living training.

Qualifications

• Graduate from a two-year college level program approved by the American Physical Therapy Association.
• Currently licensed, registered or certified in the state(s) in which practicing.
• Two (2) years’ experience, preferred.
• Acceptance of philosophy and goals of Agency.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Under PT supervision, provide prescribed PT services and adhere to agency policies.
• Provide physician prescribed PT under a plan of care established by the PT.
• Improve or minimize residual physical disabilities of the patient.
• Return the patient to optimum and productive level within their capabilities.
• Collaborate with health care personnel in care planning.
• Perform all skilled procedures as ordered by physician and per plan of care.
• Consult with PT regarding changes in treatment.
• Instruct patients and families in home programs and activities of daily living.
• Participate in inservice programs and present as assigned.
• Participate in QAPI activities as assigned.
• Attend patient care conferences as scheduled.
• Prepare medical records, progress notes and updates to care plans timely.
• Perform services planned, delegated and supervised by the PT.
• Assist in preparing clinical and progress notes.
• Participate in educating the patient and family.
