Collage Home and Community Rehabilitation Partners is seeking experienced Licensed Clinical Social Workers to provide patient-centered care in Fullerton, CA, and surrounding areas, in home and community settings. This PRN opportunity supports efficient, compassionate care across diverse neurological conditions.

Our interdisciplinary teams create integrated treatment plans and emphasize evidence-based practices to achieve durable outcomes for patients and families.

Qualifications

• Completion of a Master's degree in Social Work and state licensure as an LCSW are required
• Excellent planning, organization, writing, and time management skills are required
• Ability to communicate with families, professionals, and co-workers is a must

Responsabilités

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification
