Collage Home and Community Rehabilitation Partners is seeking Licensed Clinical Social Workers to provide patient-centered care in Fullerton, CA, and surrounding areas. This PRN role focuses on delivering services in patients' homes and communities as part of an interdisciplinary team.

Experience in neuro and complex ortho cases, strong communication with families and professionals, and a commitment to evidence-based, durable outcomes are essential.

Competenze

• Completion of a Master's degree in Social Work and state licensure as an LCSW are required
• Excellent planning, organization, writing, and time management skills are required
• Ability to communicate with families, professionals, and co-workers is a must

Mansioni

• Assess and counsel medically complex patients with individualized treatment plans.
• Coordinate resource linkage and behavior modification for adjustment to disability.
