Collage Home and Community Rehabilitation Partners seeks experienced Licensed Clinical Social Workers to provide patient-centered care in Tustin, CA, and surrounding areas. PRN positions available for clinicians who will accept patients for the duration of their stay, working with interdisciplinary teams in patients' homes and communities.

Qualifications

• Master's degree in Social Work required.

Responsibilities

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification.
