Collage Home and Community Rehabilitation Partners in Los Angeles, CA is seeking experienced Licensed Clinical Social Workers to provide patient-centered care in patients' homes and communities. PRN positions available for clinicians who will accept patients for the duration of their stay.

Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities, addressing neuro and complex ortho diagnoses with an evidence-based, durable approach.

Qualifications

• Master's degree in Social Work and state licensure as an LCSW are required
• Excellent planning, organization, writing, and time management skills are required
• Ability to communicate with families, professionals, and co-workers is a must

Responsabilités

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification
