Job summary

Advanced Home Health and Hospice is seeking a compassionate Home Health and Hospice Social Worker to provide psychosocial support to patients and families in their homes. The role involves assessment, intervention, and collaboration with an interdisciplinary team to advocate for patient needs and connect families with community resources.

Schedule: PRN, Weekdays, Weekends, As Needed. Responsibilities include assessments, care planning, counseling, resource education, and bereavement support

Qualifications

• Bachelor's in Social Work is required.
• MSW from an accredited school is preferred.
• No experience required, but elder care experience preferred.
• Knowledge of psychosocial aspects of illness, grief, end-of-life care.
• Proficiency with electronic health record (EHR) systems.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct initial and ongoing psychosocial assessments of patients and families to identify emotional, social, environmental, and financial needs.
• Develop and implement individualized care plans addressing grief, loss, coping strategies, and crisis intervention as needed.
• Provide counseling and emotional support to patients and families navigating illness progression and end-of-life decisions.
• Educate patients and families on available community resources, support groups, and advance care planning options.
• Facilitate communication between patients, families, and the interdisciplinary team to ensure a coordinated approach to care.
• Act as a patient advocate, ensuring rights and preferences are respected.
• Document all patient interactions and interventions accurately and timely in accordance with agency policies and regulatory requirements.
• Participate in interdisciplinary team meetings to discuss progress and revise care plans.
• Provide bereavement support to families following a patient's death.
• Adhere to ethical guidelines and professional standards; maintain patient confidentiality.
