Job summary

Select Specialty Hospital in Orlando seeks a PRN Case Manager. Requires active licensure as a Nurse, RT, or Social Worker and prior discharge planning or hospital experience. The role focuses on discharge planning, utilization reviews, and coordination with payors, reporting to the Director of Case Management.

Shifts are on an as-needed basis, typically weekdays 8 a.m.–5 p.m., with a minimum commitment of 6 shifts per month. Prior hospital experience is highly preferred.

Qualifications

• Current licensure in a clinical discipline (RN/LPN/LVN) or RT or Social Work per state guidelines.
• Previous inpatient hospital discharge planning or case management experience.
• Experience in acute medical case management is preferred.

Responsibilities

• Perform discharge planning and utilization reviews.
• Coordinate with payors to ensure proper reimbursement.
• Lead multidisciplinary team meetings and document care plans.
• Maintain documentation in medical records per state guidelines.
