Job summary

Agape Hospice Care is seeking an On-Call Registered Nurse (RN) to join our hospice team for weeknights. Primary duties include administering skilled nursing care in patients' homes, coordinating with an interdisciplinary team, patients, families, and referring agencies to ensure continuous, compassionate care.

The role requires an accredited nursing degree, an active RN license, CPR certification, and strong interpersonal skills to support patients and families in challenging times.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and driver’s license.
• Two years’ RN experience; hospice or home health preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office experience preferred.

Responsibilities

• Administer skilled nursing care in clients’ residences.
• Coordinate care with interdisciplinary team, patient and family.
• Coordinate care with referring agency and ensure smooth transitions.
• Provide ongoing nursing support and education to patients and families.
