Job summary

516 Visiting Nurse Association of Wisconsin, Inc. is seeking an RN for the WI Hospice Divisional - Nursing: SCT (Supportive Care Team) in the Racine/Kenosha area. This is a part-time, as-needed role with shifts ranging 8 to 12 hours on AM/PM/NOC. You must work 2 shifts per month, one of those must be a weekend shift.

Pay range $35.50 - $53.25. This role supports quality patient care through coordination with the care team and adherence to nursing standards and safety protocols.

Qualifications

• Graduate from an accredited school of nursing.
• RN license with current CPR certification.
• At least 1 year of medical nursing experience.
• Willing to work weekends/holidays and varying shifts.
• Ability to lift up to 100 pounds.

Responsibilities

• Identify communication styles and coordinate with the care team.
• Assess patient and family needs and participate in care planning.
• Develop, implement, and evaluate plans of care with resources.
• Educate patients and families using teaching protocols.
