Job summary

Horizon Hospice of Colorado is seeking a PRN Hospice RN to provide skilled nursing visits and care coordination in patients’ homes and facilities across Colorado. This role supports the clinical team during absences, PTO, and high census periods with on-call coverage as needed.

Responsibilities include patient assessments, symptom management, education for families, and collaboration with the IDT to ensure high-quality hospice care with flexible scheduling.

Qualifications

• Current Colorado RN license required.
• Minimum 1 year hospice nursing experience preferred.
• Strong clinical assessment and critical thinking skills.
• Ability to work independently and adapt to changing patient care needs.
• Valid driver's license and reliable transportation.
• Current CPR certification required.

Responsibilities

• Perform comprehensive patient assessments including physical, psychosocial, and environmental needs.
• Provide skilled nursing visits to hospice patients in homes, assisted living, SNF, and other settings.
• Monitor patient condition and communicate changes to the clinical team.
• Administer medications, treatments, and interventions as ordered.
• Provide education to patients and families regarding symptom management and care goals.
• Assist with urgent symptom management and crisis intervention when needed.
• Document patient assessments, interventions, and communication in the EMR in a timely and accurate manner.
• Collaborate with the interdisciplinary team to support continuity of patient care.
• Coordinate services including medications and equipment as appropriate.
• Participate in IDT meetings and clinical discussions as requested.
• Provide compassionate support and education to patients and families.
