Job summary

Alamo Hospice in Waco, TX is seeking a PRN Nurse Practitioner to join our growing team. You will provide high-quality hospice care with flexible hours and no on-call requirements, covering Waco, TX, and surrounding counties.

You will work in home settings, performing patient assessments, care planning, and coordination with the interdisciplinary team to ensure compassionate, patient-centered care. Prior NP licensure and RN background required.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a Nurse Practitioner in the state of practice.
• Minimum of two years of varied work experience as a professional nurse.
• Must possess current CPR Certification.
• Ability to make home visits and provide nursing care in the home environment.
• Interpersonal, verbal, and written communication skills necessary to complete job responsibilities.
• Valid driver's license and proof of insurance is required.

Responsibilities

• Determine patient eligibility for care based on State and Federal guidelines.
• Collect physical, psychological, social, and spiritual data to guide care plans with the team.
• Communicate with physicians and other staff to admit or recertify care.
• Coordinate resources to meet patient needs.
• Provide regular home visits and update the care plan as needed.
